Nate will discuss partnerships with correctional facilities and local providers\, bringing together peer mentorship\, housing and other resources\, and adapting the model in communities across Michigan. \nFollowing the presentation and Q&A\, participants will join an open discussion about local strengths\, unmet needs and opportunities for collaboration.
